What does MODANI do?
MODANI provides professional home care services to individuals who want to live in their own homes but require assistance due to a disability, chronic disease, or advanced age. MODANI is also a Provider for HOPE waiver in-home care, including the Structured Family Caregiving (SFC) program, and the VA. -
What are the HOPE waiver and SFC?
South Dakota’s Home and Community-Based Options and Person-Centered Excellence (HOPE) waiver is a Medicaid program that provides home and community-based services to people who need a nursing-facility level of care. It allows people 65 and older, and 18 and older with a qualifying disability, to live at home or in the setting that gives them most independence.What is SFC?
South Dakota’s Medicaid-funded Structured Family Caregiving (SFC) program is one of the care options available under the HOPE waiver. It allows people who need personal care at home to choose a family member who lives in the same home as their principal caregiver, who will receive a stipend for providing care.
Who is eligible for SFC?
If you are eligible for Medicaid and a HOPE waiver, and you require assistance with personal care, you’re probably eligible for SFC. To receive care from a principal caregiver who lives with you, you must be at least 65 years of age or at least 18 years of age with a qualifying disability, able to live independently with help from a home caregiver, require a nursing-facility level of care, and need one or more home care services at least once each month.
Who can receive a stipend for providing care as a principal caregiver with SFC?
A family member who lives in the same home, can pass a background check, and is the primary person responsible for providing daily care can become a principal caregiver through the CDS program. Both the principal caregiver and the person receiving care must be at least 18 years old. Principal caregivers are also required to pass an assessment for capability of providing care and to receive coaching.

How much will MODANI’s services cost me?
Our professional services can be covered by a wide range of insurance plans. In-home services provided by professionals and family members can be covered by South Dakota’s HOPE waiver Medicaid program or the VA as well. -
Do SFC caregivers need professional training or certification?
No. You must be at least 18 years of age to be paid as a principal caregiver in the SFC program, and you must pass a background screening and assessment for capability of providing care. After you pass, you must receive coaching in skills such as first aid, CPR, the Heimlich maneuver, infection control and safe personal care practices, and recognizing support needs. MODANI provides all required coaching free of charge.
