What does MODANI do?
MODANI provides professional home care services to individuals who want to live in their own homes but require assistance due to a disability, chronic disease, or advanced age. MODANI is also a provider for the VA and several Medicaid waiver programs that cover in-home care, including the Structured Family Caregiving (SFC) program. -
What is a Medicaid waiver?
A Medicaid waiver program covers services that are not included in standard Medicaid coverage. Georgia offers several Medicaid waivers that cover in-home care for seniors and people living with disabilities. These include the Community Care Services Program (CCSP), Independent Care Waiver Program (ICWP), and Service Options Using Resources in a Community Environment (SOURCE) program. What is SFC?
Georgia’s Medicaid-funded Structured Family Caregiving (SFC) program is one of the care options available under the CCSP and SOURCE waivers. It allows people who need personal care at home to choose a family member who lives in the same home as their primary caregiver. That person will be paid weekly for providing care.Who is eligible for SFC?
If you are eligible for Medicaid and an applicable waiver, and you require at least five hours of care each day, you’re probably eligible for SFC. To receive care from a primary caregiver who lives with you, you must be at least 65 years of age or have a qualifying disability if you’re younger. You must require assistance with activities of daily living but be able to live independently with help from your in-home caregiver.
Who can be paid as a primary caregiver with SFC?
A family member who lives in the same home and is biologically related to you or related through marriage (such as a son or daughter in law) can be your primary caregiver. Spouses and legal guardians are not eligible. Your primary caregiver must pass a background check, complete a brief training program, and be committed to providing daily care under the SFC program. Primary caregivers cannot be employed outside the home or run a home-based business while providing care.

How much will MODANI’s services cost me?
Our professional services can be covered by a wide range of insurance plans. In-home services provided by professionals and family members can be covered by several of Georgia’s Medicaid waiver programs or the VA. For individuals who need care but aren’t eligible for coverage, we offer affordable private payment plans. Do SFC caregivers need professional training or certification?
No. You must be at least 18 years of age to be paid as a primary caregiver in Georgia’s SFC program, and you must pass a background screening to enroll. After you pass, you must complete a state-approved training course that teaches first aid techniques, medicine management, and other caregiving skills. MODANI provides ongoing support and guidance to SFC caregivers.
